Submit the following documents:
- Utility bill
- Scope of Work (SOW) that includes:
- Details showing the areas where lights were replaced
- What existing fixtures were and what they were replaced with
- Quantities of fixtures
- Any exisitng or new controls
- Hours of Use (HOU) form
- Specification sheets - for all LEDs
- Itemized invoice - showing model numbers, quantities and individual pricing

How to Submit Documentation
To submit your application, complete your application online providing the requested documents outlined above.
